J.D., Elliot and Turk find themselves alone in the hospital night shift and face responsibilities without supervision for the first time. Luckily, they have Carla there to support and guide them through the night. Meanwhile, Dr. Cox doesn't know what to do, when he realizes that he has feelings for his ex-wife Jordan and is asked (forced actually) to introduce Dr. Kelso at a social event, where he will be receiving an award from the medical board.

Zunächst freuen sich die drei Freunde darüber, endlich einmal allein das Sagen zu haben, doch schnell stellen sie fest, dass dies auch Nachteile hat.

J.D., Elliot e Turk si lamentano continuamente in quanto non vengono mai affidati loro compiti importanti, ma all'improvviso il loro desiderio si realizza: si trovano per la prima volta da soli a gestire l'intero ospedale senza la supervisione di nessuno. I tre però scoprono ben presto quanto sia difficile prendere delle decisioni circa la salute dei pazienti. Fortunatamente c'è Carla ad aiutarli e a rasserenarli nelle loro insicurezze. Nel frattempo, il Dr. Cox pur di dimostrare a Jordan di essere cambiato (convincendola così a tornare insieme) accetta di fare un discorso d'elogio al Dr. Kelso nel corso di una premiazione ma manderà tutto a rotoli.
